90-tonne segment of bridge plunges into Fraser River

A crane tipped off its tracks on the Port Mann bridge construction project east of Vancouver on Friday morning and dropped a segment of the bridge into the Fraser River.
Bridge project spokesman Max Logan said the gantry crane, which is designed to move on rails, is secure and nobody was hurt, but a 90-tonne section of the bridge deck the crane was moving fell into the Fraser River.
The segment struck a pile as it fell and is now submerged in the river. It is unclear whether the segment can be recovered.
The bridge segments are brought in by barge and lifted into place by the special 720-tonne horizontal gantry crane, which is also called a segment launcher.
Logan said one of the supports for the gantry appears to have been crushed and the gantry pitched forward.
It is unclear how long the mishap will delay the project, but the contractor Kiewit is responsible for any costs that result.
The Trans-Canada Highway remains open on the old Port Mann Bridge and there are no reports of any injuries.
When completed, the $800-million bridge will provide 10-lane crossing of the Fraser River for the Trans-Canada Highway east of Vancouver. It is scheduled to be completed in 2013.
A 250-foot-tall crane collapsed Friday morning onto a building at a construction site at the Kent County jail complex in Grand Rapids, injuring three people, authorities said.
The jail was placed on lockdown following the incident. Kent County Undersheriff Jon Hess described the injuries to a construction worker and two Kent County jail trustees as minor, The Grand Rapids Press (http://bit.ly/ziaMEe) reported.
Both the department construction company officials said all inmates and workers were accounted for.
Construction company Owen-Ames-Kimball Co. said Friday in a statement it was investigating the cause of the accident and had no further information. Hess said it appears that a cable snapped as the crane was carrying cement.
Images from the scene showed part of the crane leaning against a building. A garage area where sheriff's vehicles are kept was damaged.
The main jail that houses inmates apparently wasn't damaged, and Hess said authorities were checking the structural integrity of that building.
The crane that fell has been used for seven months on a $27 million renovation project, Hess said.

Woman killed after crane falls on her car in Kfar Saba. Crane operator detained for questioning
A woman was killed in central Israel Sunday afternoon after a crane collapsed, crushing her car.
The woman, roughly 40 years of age, was killed as she sat in her car near a commercial center by the intersection of Hatzomet Street and Derech Hsharon.
MDA emergency first responders were called to the scene, but were forced to declare the woman dead shortly after their arrival.
Witnesses say part of a crane fell onto the woman’s car, crushing the vehicle – with the woman inside.
“We saw the vehicle crushed, and [next to it] on the street were pieces of the crane, which had fallen on the car,” said MDA’s Yarkon district chief Assi Dublinski. “Outside of the car there was a woman, about 40 years of age, lying on the ground, unconscious, who was not breathing and had no heartbeat and was suffering from serious multi-organ injuries. She had no vital signs, and after a short period of time, we were forced to declare her dead at the scene.”
Police have opened an investigation into the incident, and detained the operator of the crane for questioning.

Jakarta has been alarmed again by another collapse in a series of crane accidents that have recently taken place.
The accident happened at a girder launcher at the construction site of a quadruple track railway project in Matraman, East Jakarta. It collapsed at around 5 am on Sunday (Feb 4) local time, leaving four dead and five others injured.
It’s reported that two of the victims died on the spot while the other two died in hospital. According to project plan, the four-track railway project is expected to link Manggarai, Central Jakarta, to Cikarang, West Java.
The Transportation Ministry initiated the project while the execution was handled by state-owned construction firm Hutama Karya (HK). A statement released by the Transportation Minister Budi Karya Sumadi revealed on Sunday that his side and the National Transportation Safety Commission (KNKT) had recommended that the safety committee check the safety and serviceability of heavy equipment used in the construction project.
A series of accidents at infrastructure project sites in Jakarta and other parts of the country. The investigation results must be proven in writing, while the developer would be required to change equipment found not to meet standards.
A statement issued by Transportation Ministry's secretary-general, Sugihardjo, said all parties involved in construction projects should pay greater attention to safety. "We will continue to coordinate and support the investigation led by the Work Safety Committee, which is overseen by the Public Works and Housing Ministry," Sugihardjo said.
The first week of January saw six girder boxes at a toll road project connecting Depok, West Java, to the Antasari area in South Jakarta collapsed. No fatalities or injured victims were reported in that accident.
Two weeks later, a light rail transit (LRT) girder in Utan Kayu, East Jakarta, collapsed, injuring five workers. The MRT project has not been free from accidents, with a 3-ton parapet on Jl. Wijaya II, South Jakarta, collapsing and hitting a motorcyclist in November.
The Indonesia Construction Safety and Health Expert Association chairman Lazuardi Nurdin said various factors had caused the accident, including mechanical issues and fatigue. "The accident was related to the equipment, the launcher. But it may have also been caused by human error. The accident happened over the weekend and during normal working hours, so fatigue may have been a factor," Lazuardi said.

6:11 PM MYTPETALING JAYA: A 38-year-old Indonesian construction worker was killed when a crane collapsed at a construction site at Seksyen 7, Shah Alam.
The incident happened at 10.30am on Tuesday (Jan 2) when the boom of the crane fell, killing Matrias and injuring four others.
Shah Alam OCPD Asst Comm Shafien Mamat said the four injured workers included two Indonesians and two Bangladeshis, all in their 30s.
"They have been sent to Hospital Shah Alam for treatment," he said when contacted.
ACP Shafien said the case has been classified as sudden death.

Tower crane collapse kills 3, injures 4 workers in Yongin
Three workers were killed and four others injured after a high-rise crane collapsed at a construction site in Yongin, Gyeonggi Province, police said Saturday.
The 90-meter-tall tower crane snapped and fell to the ground at a construction site for a distribution center in Yongin, 50 kilometers south of Seoul, around 1:11 p.m., according to police.

Seven workers on the crane fell from 78 meters. Three were confirmed dead at the scene and four seriously injured were rushed to nearby hospitals.
The police said they will look into what caused the accident and whether the workers followed safety measures at the construction site.

KUALA LUMPUR: The Department of Occupational Safety and Health (DOSH) has issued a stop-work notice on construction activities at a construction site in Kampung Baru following the crane collapse incident here yesterday.
Its director-general, Datuk Ir. Mohtar Musri said legal action would be taken on quarters responsible, if it was found there was any breach of regulations under the Occupational Safety and Health Act 1994 and the Factories and Machinery Act 1967.
“At the time of the incident, a worker was handling the tower crane when suddenly the luffing boom of the tower crane collapsed and crashed on a vehicle outside the construction site.
“The incident caused minor injury on a worker and two members of the public, apart from damage to vehicles parked near the location of the accident,” he said in a statement.
He said the department viewed seriously such an accident, especially when it involved workers or members of the public.
Three people were injured and two vehicles were damaged in the incident which occurred about 1 pm.

The crane's tower came down across the street, thankfully there we no cars passing underneath at the time

A crane operator died on Friday in Lódz, Poland, when the tower crane he was operating - from the top cab - dropped its jib and then overturned rearwards.
The crane had a counterweighted base
The operator 22, said to be on his first job as a crane operator, was taken to hospital but died on Saturday from his injuries.
A report from the scene said that the jib of the crane – an old Wolff WK-91 with a counterweighted foundation and owned by crane rental company Herkules – collapsed after the mid jib suspension rods gave way.
The jib then dropped and the crane became unstable, overturning backwards, with the tower dropping across the street. The counter jib struck the ground and with the crane then going over onto its side onto a couple of cars.
The cab was not completley crushed, but the operator was badly injured.

MIAMI (Reuters) - Cranes at three South Florida high rises under construction collapsed in the face of heavy winds as Hurricane Irma ripped through the area on Sunday, days after authorities warned about dangers to cranes from the approaching storm.
No injuries were reported in any of the three collapses, and investigations would begin after the storm cleared, officials said.
Soon after one of the cranes collapsed, the chief executive of the company developing the building told Reuters he was attending the U.S. Open tennis tournament in New York when the accident occurred and had just learned about it.
“This particular crane, some of it was taken down,” Jorge Perez, chief executive of The Related Group, Miami’s largest developer, said by telephone. “They were surprised that it went down because they felt it was one of the more secure cranes, so we’re right on it.”
A video posted on Twitter showed the crane’s boom dangling above the unfinished building.
A crane at a Related Group project in Fort Lauderdale went down later on Sunday, Perez said. He had no immediate details about the incident.
High winds also snapped the boom of a crane erected on top of a Miami apartment building under construction. The project was being developed by New York-based Property Markets Group, according to The Real Deal, a South Florida real estate news website.
After the collapse, the boom was partly dangling on the side of the building, attached to the crane tower by a cable, photos on Twitter showed. Attempts to reach Property Markets Group offices in New York and Miami were unsuccessful.
Miami had been in touch with Perez, but the state of Florida and the U.S. Occupational Safety and Health Administration had jurisdiction over the cranes, City Manager Daniel Alfonso said. No one was immediately available to comment at OSHA or the governor’s office.
The National Weather Service recorded wind gusts in Miami reaching about 100 miles per hour (160 kph), with sustained winds of 50 to 60 mph (80 to 96 kph), as Irma moved up Florida’s west coast.
As Irma approached last week, Miami officials said 20 to 25 construction cranes were up across the city and that they were designed to withstand winds of 145 mph (235 kph).
It warned that the cranes had to be unpinned, so that their horizontal booms could rotate on their support columns like a weather vane.

ABBOTTABAD: Two Chinese engineers were killed when a crane crashed over them while working at China Pakistan Economic Corridor (CPEC) site Shahkot near Abbottabd.
Talking to APP District Police Officer (DPO), Syed Ishfaq Anwar said that the two Chinese engineers Sun Beef and Wang Shee Lay were working at the Hazara motorway site Shahkot when a crane crashed over them.
Both the engineers died on the spot. The bodies of both engineers were shifted to Ayub Medical Complex for medico-legal formalities.
DPO Ashfaq Anwar said dead bodies would be shifted to China after completing formalities. – APP

KUALA LUMPUR, Feb 26 — Thousands of motorists traveling along MRR2 today were caught in a massive traffic jam especially from Ulu Klang to Ampang after a crane collapsed at the site of a construction project near Dataran Ukay at about 10am today.
Bernama found that as of 1.30pm, only one of the three lanes along the route could be used as the crane was still obstructing traffic flow.
According to Selangor Traffic Investigation and Enforcement Department Head Superintendent Azman Shari’at, the traffic congestion stretched five kilometres from Ulu Klang to Ampang.
He said the consultants and contractors involved in the project were in the process of moving the crane which was obstructing traffic.
The incident also resulted in a man, 36, being injured and now being treated at the Ampang Hospital.
As at 6.20pm, two units of crane transporters were used to remove the crane that collapsed at the project site near Dataran Ukay, here this morning.
According to sources, one of the two cranes was placed near the embankment of the slope near the site of the collapsed crane while the other crane was parked on the road to provide support.
“At the moment, the project contractor was in the process of fixing a special chain to one of the wheels of the crane to carry the collapsed crane.
“In addition, a temporary platform was also being erected to place the collapsed crane and this was being implemented as soon as possible,”he told Bernama.
Traffic congestion was still being reported at the MRR2 especially from the direction of Ulu Kelang to Ampang. — Bernama
